We accept referrals from your primary care provider (PCP) or other specialists via fax at 817-358-0125 or LeadingReach. Please include any relevant lab results, imaging studies, and/or pertinent records in the referral.
We also accept self-referred patients if insurance allows (such as Traditional Medicare, certain PPO plans), as well as self-pay (“cash”) patients without insurance.
We also offer telehealth visits on a case-by-case basis.
