Referral Policy for HMOs and other plans requiring a referral

If your insurance requires a referral or authorization from your primary care physician (PCP) to see us, it is your responsibility to obtain a valid, current referral and ensure it is received by our office at least 5 business days before your appointment. A valid referral must be unexpired, issued for our practice or provider, and cover the services scheduled, including the number of visits. If your appointment is scheduled less than 5 business days in advance, we must receive the referral before your appointment date.

If we do not receive a valid referral by the deadline, your appointment will be cancelled, and you are welcome to rebook once we have received it. Please contact your PCP's office to request the referral. If you have questions about your plan's requirements, contact your insurance company.
